Dying and respawning in the same location as i died

It happens everywhere in-game. Sorry for the low quality of the video just wanted to show you the bug and not render for days, but i think you guys already know about this bug.
First death at 41sec.
And when i die in a multiple layer place then i fall through to the next level and have a visual bug, while the monsters can attack me.
