Please explain how the wall got into this state. Is it reproducable? If so, how?

I don't know actually it happens pretty rarely. 1-2 times a day. I was just trying to jump over the wall but my character ran to this place and passed without jumping.

I've tryed to login with diffirent network and computer the bug still exist.
