Title: VBO Graphical Bugs - Misplaced Blocks & Blue Glitch

Misplaced Blocks
Near the workers in the kalphite hive one of the walls seems to push outward while in VBO mode.
Using OpenGL version.

::reloadmap did not fix the issue, going to restart client and update.
Closing the client and re-opening DID fix the issue UNTIL I toggled VBO off and on again.

Blue Glitch
And the main reason I stopped using VBO -- the blue glitch.
I think this mainly occurs when there's a lot of drops in the near vicinity.

This also MIGHT be caused when another bug with VBO occurs:

Sometimes items will appear dropped twice.
Usually in adjacent blocks / diagonal blocks of each other.
Of course there's only one item but it appears twice.
As you see in the GIF above, there's 2 seeds (one below the potion too)
Once I picked the seeds up, the glitching stopped.

Thanks for the detailed bug report. I cannot reproduce the item drop issues, do you have a way of doing so? Otherwise it's pretty difficult for me to find the source and fix the bug. I also don't have the kalphite hive's wall bug, mind checking if the bug still exists there? If not, any way of reproducing it?
