Title: Fixed now, so I locked. Create ticket, input not appearing

When you are reporting a user using the ticket system it says, "The following issue, or issues, occurred while trying to post this ticket:
The following fields were not completed and need to be: Username."

Had to change field-1 to the username manually to submit the ticket.

As you can see in the photo below, there is not a input field that shows up for a user to input a username.
